Preservação da Glândula Submandibular nos doentes com CPC da cavidade oral

Resumo

Objetivo: Avaliar a possibilidade de preservação da glândula submandibular (GSM) no tratamento cirúrgico com esvaziamento ganglionar cervical, dos doentes com carcinoma pavimento-celular da cavidade oral (CPCCO). 

Desenho do Estudo: Estudo retrospetivo. 

Material e Métodos: Revisão do processo clínico dos doentes com CPCCO submetidos a tratamento cirúrgico primário entre 2010 e 2016, comparando a sobrevida e recidiva tumoral entre os doentes em que houve preservação da GSM (n=26) e aqueles em que foi removida (n=51). 

Resultados: Não se verificou diferença estatisticamente significativa na sobrevida (p=0,121) ou na taxa de recidiva loco-regional (p=0,475) entre os grupos. 

Conclusões: A recidiva loco-regional e a sobrevida destes doentes não parecem ser condicionadas pela exérese da GSM. Estes dados sugerem poder ser viável a sua preservação, considerando não se tratar de um estadio tumoral avançado nem apresentar evidencia clínica de invasão, podendo assim evitar-se eventuais complicações decorrentes da sua exérese e o compromisso da qualidade de vida dos doentes.
