Abstract

47-year-old female who presented with a sensation of clenching in the cheek and right upper lip for about eight months. Upon examination, hypoesthesia in the right V2 was detected. Using endonasal endoscopy, a slightly reddish mamelonated tissue was observed above the right tubal cuff. Suspecting a vascular neoformation, it was decided not to do a biopsy and a radiological examination using computed tomography (CT) was ordered. A solid space-occupying lesion was described, centred in the pterygopalatine fossa/right chewing space with an expansive reaction on the surrounding cortical bones. The examination was completed with magnetic resonance imaging (MRI) which reported an expansive process in which branching tubuliform images with contrast uptake were identified and were compatible with hypertrophied vessels. The suspected radiological diagnosis was trigeminal schwannoma. Surgical treatment was indicated, opting for expanded endonasal, transmaxillary, transpterygoid approach combined with midfacial degloving. Complete exeresis of the lesion was successful. Anatomopathological examination reported a benign paraganglioma. Follow-up was unremarkable with absence of persistent/recurrent lesion.
