Tympanostomy tubes complications in children

Abstract

Introduction: Despite the technical simplicity of the insertion of tympanostomy tubes, as a surgical procedure, it’s not free of risks. The most frequent postoperative complications include the presence of otorrhea, tube lumen obstruction, granulation tissue formation, premature extrusion, tube migration into the middle ear, myringosclerosis, atrophy or retraction pocket formation at the site of myringotomy or tympanic perforation after tube extrusion.

Methods: Sample: children undergoing myringotomy in the Hospital Pediátrico (CHUC) in the year of 2013. The following data were collected: age, gender, indication for myringotomy, documented postoperative complications and follow-up time.

Results: There were 169 children undergoing myringotomy. Otitis media with effusion was the cause for surgery in 97,6% of cases. A total of 215 grommets were inserted and the most frequent complications were: transient otorrhea (12,6%), recurrent otitis media with effusion after grommet extrusion (11,2%), grommet lumen obstruction (4,7%), tympanic perforation after grommet extrusion (2,3%), recurrent otorrhea (1,9%), posterior diagnosis of cholesteatoma (1,4%) and recurrent acute otitis media after grommet extrusion (0,4%).

Conclusions: We found complication rates to be similar to larger studies, except for the existence of cholesteatoma after tube extrusion (which was higher than expected in the present study).
