Abstract

Positron Emission TomographyIComputed Tomography (PETI CT) scan is considered a last resource diagnostic tool for the head and neck occult primary tumour. Its ability to identify the occult tumour location may lead to a more adjusted treatment, avoiding undesired side e!fects or invasive procedures.

The authors study the current diagnostic relevance of PETICT scan in the management of patients with head and neck occult primary tumour based on a clinical case.

A 50 year old male presented with a cervical mass. Fine needle biopsy revealed squamous cell carcinoma metastasis followed by upper airway endoscopy, several imaging studies, bronchoscopy, randomized biopsies of the Waldeyer ring, tonsillectomy and whole body PETICT scan. The workup was inconclusive.  The treatment was ipsilateral modified radical neck dissection including level I through level V. Intraoperatively we had the clinical impression that the primary tumour was located at the submandibular gland.

The definitive diagnosis was reached by histology.
