Association of gastro-esophageal reflux with chronic sinonasal disease in adult: Relationship or coincidence?

Abstract

Introduction: Chronic rinosinositis(CRS) is estimated to affect almost 20% of the Portuguese population. There are patients whose symptoms are resistant to the medical and surgical treatments, frequently the reasons for this are unknown. There is the hypothesis of a possible relation between the gastroesofagic reflux (GER) and the above mentioned cases of rinosinositis resistant to treatment. The validation of this hypothesis rests in three assertions: 1) a higher prevalence of concurrency of GER and CRS, comparatively to the general population; 2) the existence of a physiopathological mechanism that explains this relation; 3) improvement of the clinical nasosinusal symptoms attributed to reflux after antireflux treatment.

Materials and Methods: The articles analysed in this revision were identified through a systematic search, performed at PubMed and the terms of search used were “reflux”, “antireflux therapy”, “proton pump inhibitor”, “sinusitis”, and “rhinosinusitis” sinonasal.

Results: The PubMed search resulted in 264 articles, from which were excluded all of those that were not originals, those unpublished in English or the ones that did not wish a study conducted in humans. A total of 18 articles were analysed.

Conclusions: The articles published so far offer a weak and inconsistent support to a possible relation between the CRS and the GER. For the future, it is essential to perform randomized studies with proper control, based on standardized, precise and validated criteria.
