Abstract

Objectives: The laryngotracheal stenosis remains a challenge in reconstructive surgery of the larynx. The aim of this study is to analyze different treatment options depending on the type of stenosis.

Methods: We performed a retrospective analysis of seven consecutive patients with acquired laryngotracheal stenosis followed at our institution between the years of 2000 and 2010. The surgical techniques included endoscopic dilatation, segmental resection, laryngotracheal reconstruction with costal cartilage and lyophilized bone and tracheoplasty with Montgomery tube placement.

Results: In all cases, except two, the treatment was effective in controlling the stenosis, and the patients are asymptomatic and completely decannulated. In two cases there was a need for further treatment with endoscopic dilation. No major complications were detected.

Conclusions: The importance of a multidisciplinary approach. An initial conservative treatment was effective in most cases. Laryngotracheal reconstruction with costal cartilage and bone seems to be more effective in the treatment of complex laryngotracheal stenosis.
