Abstract

The most frequent cause of petrous apex destruction is a secondary process from either contiguous lesions or metastases.

The primary lesions intrinsic to the apex are less common but not rare.

These lesions are clinically silent for long periods with the patients often complaining of vague and non-specific symptoms that delay diagnosis.

lmage studies, CT and MR have revolutionized the preoperative work-up of petrous apex lesions.

Only histopathologic examination after lesion excision allows on accurate diagnosis of this kind of pathology.

We discuss the probable diagnosis: Mucocele; Cholesteatoma and Cholesterol Granuloma.

Petrous apex lesions are frequently detected accidentally and imaging exams are fundamental for their diagnosis.

Treatment depends on symptoms, dimensions and etiologic suspicion and most times is surgical.

Only surgery con give us the certain diagnosis.
