Acute calcific tendonitis of the Longus Colli muscle: A disease to be remembered

We present a case of an acute calcific tendonitis of the longus colli muscle, aim to alert to this benign and self-limiting condition and the possible consequences of its non-recognition. We describe a 57-year-old female presenting with odynophagia and dysphagia with 4 days of evolution, associated with neck pain and limited cervical movements. Objectively, she was subfebrile and evidencing pain and limited flexion and cervical rotation movements to the right. Nasofibrolaryngoscopy revealed a bulging, non-pulsatile mass of the posterior pharyngeal wall, from the nasopharynx to the level of the epiglottis. Based on clinical issues, suspecting that a retropharyngeal abscess was present, empirical antibiotics were initiated. A cervical CT scan was requested, which, together with the clinical backgroud, established the diagnosis of acute calcific tendonitis of the longus colli muscle. The patient was discharged, under conservative measures, with complete resolution of symptoms after 15 days.
