Biphenotypic Sinonasal Sarcoma (BSS) is a rare tumor, exclusive to the sinonasal tract, described relatively recently and which is known for its peculiar features. It is a low-grade malignant sarcoma, with cells showing features of neural and myogenic differentiation, which tends to recur locally and does not metastasize. We present the case of a 36-year-old man who presented nasal respiratory insufficiency due to a unilateral mass. After surgery, a low-grade mesenchymal tumor was diagnosed. During follow-up, after 6 years, a 3-cm sphenoid tumor was detected, which after excision was confirmed as biphenotypic sinonasal sarcoma.
