Biópsia do gânglio sentinela no carcinoma pavimentocelular da língua

Resumo

Objetivo: Reportar os resultados da biópsia do gânglio sentinela (BGS) no carcinoma pavimentocelular (CPC) da língua.

Desenho do Estudo: Estudo retrospetivo observacional.

Material e Métodos: Avaliação dos processos clínicos dos doentes com CPC da língua cT1-2N0 submetidos a BGS. Foi obtida uma amostra de 23 doentes. A histologia da peça de esvaziamento ganglionar cervical (EGC) constituiu o gold standard para diagnóstico de metástases ganglionares cervicais. O período médio de seguimento foi de 29.7 meses.

Resultados: Dos 47 gânglios sentinela colhidos, 17% foram positivos para metástases de CPC, correspondentes a 26.1% dos doentes. Dos 18 doentes submetidos a EGC, 22.2% apresentavam metástases de CPC. Nesta amostra, a BGS apresentou sensibilidade de 75% e valor preditivo negativo (VPN) de 83.3%.

Conclusões: A BGS demonstrou valores elevados de sensibilidade e de VPN nesta série. A utilização mais sistemática desta técnica permitirá evitar EGC desnecessários e aumentar a sua exatidão
