Recidiva da papilomatose laríngea após tratamento cirúrgico

Objetivos: Analisar série de casos de papilomatose laríngea (PL) no Hospital Garcia de Orta. Rever técnicas cirúrgicas e terapêuticas adjuvantes atualmente existentes.
Métodos: Estudo retrospetivo dos doentes com PLentre 2009 e 2014. Analisados parâmetros epidemiológicos, clínicos e terapêuticos.
Resultados: Registaram-se 17 casos de PL, 88,2% do sexo masculino com média de idade 60,6±9,2 anos, 82% apresentavam hábitos tabágicos e 47% etanólicos. O sintoma de apresentação mais frequente foi a disfonia em 88% dos casos. A recidiva foi de 43,8%, com tempo médio de primeira recidiva de 13 meses, tendo sido a microcirurgia laríngea com remoção das lesões a frio a abordagem de eleição (75%). Em 2 casos, observou-se evolução para carcinoma pavimento-celular.
Conclusões: A PL apresenta comportamento recorrente. O tratamento cirúrgico continua a ser o pilar na abordagem, com uma nova tendência em relação ao microdebridor. É fundamental aumentar o tempo entre recidivas, considerando as terapêuticas adjuvantes disponíveis atualmente.
