Cirurgia de Schwannoma do nervo vestibular ou cirurgia de preservação do nervo facial?

Os autores revêm a evolução da estratégia cirúrgica referente ao schwannoma do nervo vestibular. Se, no passado, a exérese total do tumor era o objectivo principal da cirurgia, no presente esta abordagem mudou de modo significativo. A preservação do nervo facial constitui um dos principais objectivos da cirurgia, assim como a exérese do tumor. É dado especial ênfase ao tratamento dos tumores de grandes dimensões (grau 3 e 4 da Escala de Koos) e sua relação com a qualidade de vida do doente no pós operatório.
É descrita uma técnica de imagem inovadora em Portugal realizada no Hospital Lusíadas, em Lisboa, utilizada pela Unidade Funcional de Cirurgia da Base do Crânio e Neurorrinologia do Hospital de Egas Moniz, em Lisboa: a Tratografia do nervo facial por Difusão de Imagem, por Ressonância Magnética. Esta técnica permite conhecer, no pré operatório, o trajecto do nervo facial no seu segmento cisternal e sua relação anatómica com o tumor localizado ao ângulo ponto-cerebeloso.
