Ácido tranexâmico versus irrigações com soro quente na cirurgia endoscópica nasosinusal: uma revisão e meta-análise

Resumo

Objetivos: A hemorragia intraoperatória pode comprometer os resultados da cirurgia endoscópica nasosinusal (CENS). O ácido tranexâmico (ATX) e a irrigação com soro quente (ISQ) demonstraram ser úteis na redução da perda sanguínea na CENS. O objetivo desta revisão foi o de comparar os resultados destas duas modalidades na CENS.
Desenho do estudo: Revisão sistemática e meta-análise
Material e Métodos: A investigação incluiu as bases de dados da PUBMED e Cochrane. Foi efetuada uma meta-análise que explorou os resultados do ATX e da ISQ na CENS.
Resultados: Doze trabalhos cumpriram os critérios de elegibilidade para meta-análise. A ISQ apresentou uma redução mais significativa na perda sanguínea comparativamente ao ATX (p=0,01). Não foi encontrada diferença estatisticamente significativa entre ATX e ISQ quanto à qualidade do campo cirúrgico (p = 0,99) e à duração da cirurgia (p=0,24).
Conclusões: Este estudo sugere eficácia semelhante entre ATX e ISQ na melhoraria das condições operatórias na CENS, com vantagem da ISQ na perda sanguínea volumétrica.
